Adoption Pre-Screen

You must complete this application in full before being considered to adopt one of our available dogs.

This is the first of two applications adopters are required to fill out. This form is a pre-screen so that we can determine if you meet our basic requirements for adoption. If you do, our application committee will send you the link to the full application. This process is in place to ensure that applicants are serious about adopting, and to avoid having applicants fill out a full adoption application if they do not meet our basic requirements.

  • Please provide your landlord’s contact information (name and phone number - not email). We suggest that you notify your landlord that we will be calling to confirm.
  • (size of area, height & type of fencing, shelter, etc.)
  • Please list which available dogs you are interested in adopting.
  • Include name, species, breed, age, and whether they are spayed or neutered
  • 1. the application process takes an average of 3-4 weeks, but may be more depending on the circumstances. 2. the primary form of communication is EMAIL, and my email address at the top of this form is correct. 3. applying for a specific dog in no way guarantees that dog will be available upon approval, or that I will be selected as the adopter.
